Explanation of Services

This term refers to “Activities of Daily Living”. When a client requires assistance with ADL’s this includes such tasks as bathing, dressing, personal hygiene and in certain circumstances incontinent care.

This term refers to a client who requires assistance with ADL’s twenty four hours a day to remain living independently in their home. Depending on the needs of the client, it could be a home health aide or a certified nursing assistant, that lives in the clients home providing assist with ADL’s, light housekeeping (dishes,laundry and occasional vaccuming), meal preparation, shopping and transportation to MD appointments. This also includes medication reminders, if required. Modifications to care plan can be done at any time by the client or family as needed.

It is very important for clients to maintain a certain level of “socialization”. Transportation to senior centers, religious services or any activity client requests can be arranged. This will help in maintaining a certain level of “personal wellbeing” if you will, and reduce risks associated with isolation

This includes assisting the client into the office and relaying any pertinent medical information to the MD and then back to the family members and/or licensed nurse.

This refers to tasks such as vacuuming, dishes, dusting and assist with daily personal laundry. Changing of bed linens will be done on a weekly basis or as otherwise requested by client.
